Dehydration and Heart Palpitations: Can Not Drinking Enough Cause Them?

Losing more fluid than you replace reduces circulating blood volume. The heart may respond by beating faster to maintain blood flow, which can feel like racing, pounding or unusually noticeable heartbeats. Fluid loss may also affect blood pressure and electrolytes.

Yes. Dehydration can cause a faster heart rate and may make your heartbeat feel pounding or unusually noticeable. Fluid loss reduces circulating blood volume, and the heart may compensate by beating faster. Dehydration can also contribute to low or orthostatic blood pressure and, when substantial fluid loss is accompanied by electrolyte changes, may increase susceptibility to rhythm symptoms in some people. Mild dehydration may improve with appropriate fluid replacement, but persistent, markedly irregular or recurrent palpitations should not automatically be attributed to dehydration.

When Dehydration With Palpitations May Be an Emergency

Seek urgent or emergency medical care if palpitations occur with:

  • fainting or loss of consciousness;
  • collapse;
  • confusion or markedly reduced alertness;
  • severe or sudden shortness of breath;
  • significant chest pain or pressure;
  • severe dizziness or near-fainting;
  • a sustained very rapid or markedly irregular heartbeat;
  • very little or no urine;
  • rapid breathing with severe weakness;
  • signs of shock such as cold clammy skin or poor responsiveness; or
  • rapidly worsening symptoms.

Severe dehydration can be life-threatening and may require intravenous fluid and medical monitoring rather than simply drinking more water at home.

Why Can Dehydration Cause Heart Palpitations?

Water makes up a large part of the circulating volume within your blood vessels.

When your body loses more fluid than you replace, circulating blood volume can fall.

To maintain blood flow to the brain, muscles and other organs, the body may respond by:

  • increasing heart rate;
  • constricting blood vessels;
  • changing kidney handling of water and sodium; and
  • activating hormonal and autonomic responses that help preserve circulation.

The resulting faster or stronger heartbeat may be experienced as palpitations.

Often the rhythm is sinus tachycardia.

In uncomplicated dehydration, the heart’s normal pacemaker may simply speed up appropriately in response to lower circulating volume. This is different from assuming that dehydration has produced an electrical arrhythmia such as atrial fibrillation or SVT.

Can Dehydration Trigger a Real Arrhythmia?

It may contribute in susceptible people, but the relationship requires careful wording.

The American Heart Association lists dehydration among factors that may contribute to episodes of supraventricular tachycardia.

Fluid loss can also occur together with changes in potassium, sodium or magnesium, particularly after substantial vomiting, diarrhea or sweating.

These factors can affect cardiac electrical activity.

Do Not Assume Every Dehydration-Related Palpitation Is an Arrhythmia

A faster pulse from reduced circulating volume is much more straightforward than proving that fluid loss directly caused AF, SVT or another arrhythmia.

If the heartbeat is very irregular, begins and ends abruptly, persists after rehydration or repeatedly returns, an ECG or rhythm monitor may be needed to identify what is actually happening.

What Are the Signs of Dehydration?

Mild to moderate dehydration may cause:

Thirst and dry mouth

Thirst and a dry or sticky mouth are common early clues that fluid replacement has not kept pace with fluid loss.

Dark or reduced urine

Urinating less than usual or producing darker urine can indicate that the kidneys are conserving water.

Dizziness

Reduced circulating volume can contribute to lightheadedness, particularly when standing.

Fatigue and weakness

Dehydration can make exercise and normal daily activity feel more difficult.

Headache

Headache may accompany fluid loss, heat exposure or illness.

Muscle cramps

Cramping may occur after substantial sweating or fluid and electrolyte loss.

More severe dehydration can cause rapid heartbeat, rapid breathing, confusion, very low urine output, fainting or shock.

What Do Electrolytes Have to Do With Palpitations?

Electrolytes are electrically charged minerals that help regulate many functions, including the heart’s electrical activity.

Important examples include:

  • sodium;
  • potassium;
  • magnesium; and
  • calcium.

Significant vomiting, diarrhea or prolonged heavy sweating can alter fluid and electrolyte balance.

Do Not Guess Which Electrolyte Is Low

Palpitations are not proof of potassium or magnesium deficiency.

Both low and high electrolyte levels can affect cardiac function. When substantial losses or persistent symptoms are present, blood testing is safer than taking large amounts of potassium or magnesium on your own.

Exercise, Sweating and Dehydration Palpitations

Exercise naturally increases heart rate.

When exercise is prolonged, particularly in hot or humid weather, sweating increases fluid loss.

If that fluid is not replaced, the heart may need to work harder to maintain circulation.

You may notice:

  • a higher-than-usual heart rate during recovery;
  • pounding heartbeats;
  • thirst;
  • dry mouth;
  • dizziness when standing;
  • weakness;
  • muscle cramps; or
  • reduced exercise performance.
Recovery should move in the right direction.

If resting in a cool environment and replacing fluids does not improve a persistent fast or irregular heartbeat, do not continue to assume dehydration is the cause.

What About Heat Exhaustion and Heat Stroke?

Hot weather can increase fluid loss through sweating.

Heat illness may develop when the body cannot adequately compensate.

Severe dehydration can cause:

  • rapid heartbeat;
  • dizziness;
  • rapid breathing;
  • weakness;
  • confusion; and
  • loss of consciousness.

Confusion or Loss of Consciousness in the Heat Is Not a Simple Hydration Problem

Severe neurological symptoms, collapse or very high body temperature after heat exposure require emergency medical care because heat stroke can be life-threatening.

Vomiting, Diarrhea and Heart Palpitations

Gastrointestinal illness can cause substantial losses of both water and electrolytes.

The combination of:

  • vomiting;
  • diarrhea;
  • poor fluid intake;
  • fever; and
  • reduced food intake

can produce dehydration more quickly than simply forgetting to drink water.

Palpitations may occur because of:

  • reduced circulating volume;
  • a compensatory increase in heart rate;
  • fever;
  • electrolyte disturbances; or
  • the physiologic stress of illness.
Repeated vomiting or diarrhea may require more than plain water.

When meaningful fluid and electrolyte losses are occurring, an appropriate oral rehydration solution may be more useful than simply drinking large amounts of plain water. Severe dehydration, inability to keep fluids down or reduced responsiveness needs medical care.

Alcohol, Hangovers and Dehydration

Alcohol can contribute to increased urination, poor sleep, vomiting and reduced fluid intake.

A hangover may therefore involve some degree of dehydration.

However, dehydration should not be used as the only explanation for alcohol-associated palpitations.

Alcohol itself can affect cardiac electrophysiology and can trigger atrial fibrillation in susceptible people, particularly after heavier intake.

Can Dehydration Cause Low Blood Pressure and a Racing Heart?

Yes.

Dehydration lowers circulating volume. In some people this lowers blood pressure while the heart speeds up to maintain blood flow.

Symptoms may become particularly noticeable after standing:

  • dizziness;
  • lightheadedness;
  • blurred or fading vision;
  • weakness;
  • palpitations; or
  • fainting.

This can resemble or contribute to orthostatic hypotension.

Blood Pressure and Heart Rate Should Be Interpreted Together

A person with fluid loss may have a faster pulse and lower pressure, but similar symptoms can also arise from blood loss, medication effects, an arrhythmia or another medical condition.

How Can You Rehydrate Safely?

For mild dehydration in an otherwise healthy adult, replacing fluids gradually is usually reasonable.

Water

Water is appropriate for many cases of mild dehydration caused by inadequate drinking or ordinary daily fluid loss.

Oral rehydration solutions

When dehydration follows significant vomiting or diarrhea, oral rehydration solutions can replace both water and electrolytes in proportions designed for gastrointestinal fluid loss.

After sweating

Rehydration needs depend on exercise duration, heat, sweat losses, diet and individual health.

Most people do not need to take high-dose potassium or magnesium supplements simply because they exercised.

More Water Is Not Always Better

Drinking very large amounts of water rapidly can itself disturb sodium balance.

People with heart failure, significant kidney disease, cirrhosis or a medically prescribed fluid restriction should follow individualized fluid advice rather than automatically increasing intake.

How Quickly Should Palpitations Improve After Drinking Water?

There is no reliable fixed timetable.

Improvement depends on:

  • how dehydrated you are;
  • why the fluid was lost;
  • whether electrolyte abnormalities are present;
  • whether fever or illness continues;
  • whether you can absorb and retain oral fluids; and
  • whether dehydration is actually responsible for the heartbeat symptoms.

Mild symptoms from modest fluid loss may settle as hydration and circulation recover.

A persistent very fast or irregular rhythm should not be managed by repeatedly drinking more water and waiting indefinitely.

Can Drinking Water Stop an Arrhythmia?

Not reliably.

Rehydration can remove dehydration as a physiologic trigger and may normalize sinus tachycardia caused by reduced blood volume.

But water is not a rhythm-conversion treatment for:

  • atrial fibrillation;
  • atrial flutter;
  • SVT;
  • ventricular tachycardia; or
  • another persistent electrical arrhythmia.
Improvement after water does not prove the diagnosis either.

Palpitations can stop spontaneously. If episodes repeatedly recur, documenting the rhythm provides stronger evidence than assuming water “cured” the arrhythmia.

When Might Medical Testing Be Needed?

Testing may be appropriate when dehydration appears substantial, the cause is uncertain or the heartbeat symptoms remain concerning.

Vital signs

Heart rate, blood pressure, temperature, breathing rate and oxygen saturation can help assess the severity of illness.

Orthostatic measurements

When symptoms occur after standing, clinicians may compare heart rate and blood pressure while lying down and after standing.

Blood tests

Depending on the situation, tests may include:

  • sodium;
  • potassium;
  • kidney function;
  • blood glucose;
  • magnesium when clinically appropriate;
  • blood count; and
  • other tests directed by the cause of fluid loss.

ECG

An ECG can determine whether the fast heartbeat is sinus tachycardia or another rhythm if the symptoms are occurring during the recording.

A normal ECG after the episode does not exclude an intermittent arrhythmia.

If episodes repeatedly recur despite adequate hydration, a Holter, patch or event monitor may be considered depending on symptom frequency.

When a patient tells me, “I think my palpitations are from dehydration,” I first ask what caused the fluid loss. A long hot day with heavy sweating is different from three days of vomiting and diarrhea. I also ask about dizziness when standing, urine output, fever and medicines such as diuretics. Mild dehydration can certainly make the heart beat faster, and replacing fluid may help. But if the rhythm feels clearly irregular, continues after hydration or keeps returning without an obvious reason for fluid loss, I do not stop the evaluation there. We may need an ECG, blood tests or longer rhythm monitoring to find out what is actually happening.

When Should You Make a Medical Appointment?

Arrange medical evaluation when:

  • palpitations keep returning without an obvious reason for dehydration;
  • the heartbeat remains fast after rest and reasonable fluid replacement;
  • the rhythm feels persistently irregular;
  • you repeatedly become dizzy when standing;
  • you have persistent vomiting or diarrhea;
  • you cannot keep fluids down;
  • urine output has fallen significantly;
  • you take diuretics or other medicines affecting fluid balance;
  • you have kidney disease or heart disease;
  • you have recurrent weakness or breathlessness; or
  • symptoms are becoming more frequent or severe.

Urgent Warning Signs

Seek urgent or emergency medical care for:

  • fainting or collapse;
  • significant chest pain or pressure;
  • severe shortness of breath;
  • severe dizziness or near-fainting;
  • confusion or reduced responsiveness;

  • cold clammy skin or signs of shock;
