A cough and a pounding, fluttering or skipped heartbeat can occur at the same time for several reasons. Sometimes coughing simply makes the heartbeat more noticeable. In other cases, respiratory illness, medicines, heart failure, a lung problem or — rarely — an arrhythmia may connect the two symptoms.
Heart palpitations and cough can occur together without one directly causing the other. A respiratory infection, fever, asthma, albuterol or another bronchodilator, stimulant-containing cold medicines, smoking, anxiety, heart failure and pulmonary embolism are among the possibilities. Forceful coughing can temporarily alter chest pressure and heart-rate awareness, and rare cases of cough-triggered arrhythmia have been reported. Rarely, premature beats or another arrhythmia may itself trigger a cough. Seek urgent care for cough and palpitations with severe shortness of breath, chest pain, fainting, coughing blood or rapidly worsening symptoms.
When Cough and Palpitations Need Urgent Attention
Seek urgent or emergency medical care when palpitations and cough occur with:
- severe or sudden shortness of breath;
- significant chest pain or pressure;
- fainting or loss of consciousness;
- severe dizziness or near-fainting;
- coughing up blood;
- a sustained very rapid or markedly irregular heartbeat;
- one-sided leg swelling or pain together with sudden breathlessness;
- pink or blood-streaked frothy sputum with severe breathlessness;
- collapse; or
- rapidly worsening symptoms.
These combinations may occur with pulmonary embolism, acute heart failure, a serious arrhythmia or another urgent cardiopulmonary condition.
What Does It Mean to Have Heart Palpitations and a Cough Together?
Palpitations are sensations of unusually noticeable heartbeats. They may feel like:
- racing;
- fluttering;
- pounding;
- skipped beats;
- extra beats; or
- an irregular heartbeat.
A cough is a protective airway reflex. Most coughs originate from the respiratory tract, upper airway, medications or reflux rather than from the heart’s electrical system.
When coughing and palpitations occur at the same time, there are three main possibilities:
- the cough changes how the heartbeat feels;
- one underlying condition causes both symptoms; or
- rarely, an abnormal rhythm and cough directly trigger one another.
If you cough and then notice one forceful heartbeat, that is different from an ECG showing that every cough consistently triggers an atrial tachycardia. Repeated symptoms may require rhythm monitoring to establish what is actually happening.
Can Coughing Cause Heart Palpitations?
A forceful cough changes pressure inside the chest and briefly affects blood return to the heart and the autonomic nervous system.
Because of these pressure changes, some people become more aware of a forceful beat, skipped sensation or change in pulse immediately after coughing.
This does not necessarily mean the cough produced a dangerous arrhythmia.
Cough-Triggered Arrhythmias Exist — But They Are Rare
Medical literature includes rare reports of premature atrial beats, premature ventricular beats and atrial tachycardia triggered by coughing.
These unusual cases should not be generalized to everyone who notices a thump after coughing. A reproducible relationship generally needs to be documented on ECG or ambulatory rhythm monitoring.
Can Heart Palpitations or an Arrhythmia Cause a Cough?
Rarely, yes.
Case reports and small clinical observations describe people in whom premature beats or other arrhythmias appeared immediately before a repeated dry cough.
Proposed explanations include reflex nerve pathways and temporary hemodynamic changes involving the heart and pulmonary circulation.
However, arrhythmia-induced cough appears uncommon.
Asthma, upper-airway problems, respiratory infection, GERD, smoking, ACE-inhibitor medication and lung disease are all much more familiar explanations for cough. Heart failure is another important cardiovascular cause.
What Can Cause Cough and Heart Palpitations Together?
Respiratory infection and fever
Viral or bacterial respiratory illness can cause cough, fever, dehydration and physical stress. Fever and dehydration may increase the heart rate, making palpitations more noticeable.
Asthma
Asthma can cause coughing, wheezing, chest tightness and shortness of breath. During breathing difficulty, the heart may also beat faster.
Albuterol and other bronchodilators
Short-acting beta-agonists such as albuterol can relieve bronchospasm but may also cause shakiness, rapid heart rate or palpitations in some people.
Do not stop prescribed asthma treatment on your own. Persistent heartbeat symptoms should be discussed with the prescribing clinician.
Cold and cough medicines
Some decongestants contain stimulants such as pseudoephedrine. These can cause nervousness, a faster pulse or a pounding or irregular heartbeat in susceptible people.
Smoking and nicotine
Tobacco smoke irritates the airways and commonly causes cough. Nicotine also stimulates the cardiovascular system and may increase heart rate or heartbeat awareness.
Anxiety and hyperventilation
Anxiety may cause a racing heartbeat, chest tightness, throat sensations and altered breathing. Some people repeatedly cough or clear their throat when anxious.
New cardiopulmonary symptoms should not automatically be labeled anxiety.
GERD
Gastroesophageal reflux is a recognized cause of chronic cough. Palpitations may occur independently after meals or because of another trigger, but GERD itself should not be assumed to be causing an arrhythmia.
Heart failure
Heart failure can produce cough, wheezing, shortness of breath and a rapid or irregular heartbeat, particularly when fluid accumulates in or around the lungs.
Pulmonary embolism
A blood clot blocking an artery in the lung may cause sudden breathlessness, cough, chest pain, rapid or irregular heartbeat, dizziness or fainting.
ACE-inhibitor medicines
Medicines such as lisinopril, ramipril or enalapril can cause a persistent dry cough in some patients.
If palpitations occur at the same time, do not assume the ACE inhibitor caused both symptoms. The heartbeat symptoms may need separate evaluation.

Can an Asthma Inhaler Cause Heart Palpitations?
Yes. Albuterol and similar short-acting bronchodilators stimulate beta receptors to open the airways.
Common effects can include shakiness and nervousness, while some people notice a faster pulse, pounding or fluttering heartbeat.
If palpitations repeatedly occur immediately after using your inhaler, mention the pattern to the clinician managing your asthma.
Breathing difficulty itself can be dangerous. Use prescribed treatment according to your asthma plan and discuss recurrent side effects with your healthcare professional so the technique, dose and treatment plan can be reviewed.
Can Cold and Cough Medicine Cause Palpitations?
Yes, depending on the ingredients.
Some combination cold products contain a decongestant such as pseudoephedrine, which can increase heart rate and produce pounding or irregular heartbeat symptoms.
Multiple cold remedies can also contain overlapping ingredients, so read product labels carefully.
If you have an arrhythmia, significant cardiovascular disease or repeatedly experience palpitations from a decongestant, ask a clinician or pharmacist which products are appropriate for you.
Could Cough and Palpitations Be a Sign of Heart Failure?
They can be.
Heart failure may cause fluid to back up into the lungs and airways. Symptoms can include:
- shortness of breath with activity;
- breathlessness when lying flat;
- waking at night short of breath;
- persistent dry or moist cough;
- wheezing;
- leg or ankle swelling;
- rapid weight gain from fluid retention;
- fatigue;
- reduced exercise tolerance; and
- rapid or irregular heartbeat.
“Cardiac Asthma” Is Not Regular Asthma
The term cardiac asthma refers to coughing or wheezing caused by left-sided heart failure and fluid accumulation rather than inflammation of the airways.
This distinction matters because asthma and heart failure require different treatments.
When Could Cough and Palpitations Mean Pulmonary Embolism?
Pulmonary embolism is an important emergency diagnosis because a clot obstructing blood flow through the lungs can affect both breathing and cardiovascular function.
Possible symptoms include:
- sudden unexplained shortness of breath;
- sharp chest pain, often worse with a deep breath or cough;
- rapid or irregular heartbeat;
- cough;
- coughing blood or blood-streaked mucus;
- dizziness or fainting;
- excessive sweating;
- one-sided leg swelling or pain; and
- low blood pressure in severe cases.
Do Not Wait at Home if Pulmonary Embolism Is a Real Possibility
Sudden unexplained breathlessness with chest pain, coughing blood, fainting or a very fast heartbeat requires urgent medical assessment, particularly when there are blood-clot risk factors.

What if Every Cough Is Followed by a Skipped Beat?
A consistent relationship is worth mentioning to your clinician.
Occasional forceful heartbeat awareness after coughing may simply reflect changes in chest pressure or a premature beat.
But if:
- every cough is followed by the same skipped or racing sensation;
- the symptoms happen repeatedly throughout the day;
- the cough and heartbeat seem precisely linked;
- you feel dizziness or near-fainting; or
- the symptoms have persisted for weeks,
ambulatory ECG monitoring may help establish whether premature beats or another rhythm correlate with the cough.
Rare Arrhythmia-Triggered Cough Should Be Proven, Not Assumed
The strongest evidence comes when ECG or Holter monitoring shows that cough repeatedly follows the same premature beat or arrhythmia and the cough improves when that rhythm is successfully treated.
Can Coughing Stop Heart Palpitations?
Do not use repeated forceful coughing as a general treatment for heart palpitations.
Internet advice sometimes calls this “cough CPR,” but this is misleading. The American Heart Association does not recommend routine cough CPR for people outside monitored medical settings.
In very specific hospital situations, a conscious patient developing a witnessed arrhythmia while continuously monitored may occasionally be coached to cough temporarily while definitive treatment is prepared.
That situation is completely different from treating unexplained palpitations at home.
Coughing Is Not a Substitute for Emergency Treatment
If you develop serious chest pain, severe breathlessness, fainting, collapse or another emergency symptom, seek emergency help rather than attempting repeated forceful coughing.
How Do Doctors Evaluate Cough With Heart Palpitations?
Evaluation depends on whether the cough appears primarily respiratory, medication-related, cardiovascular or potentially rhythm-related.
History
Your clinician may ask:
- which symptom starts first;
- whether every cough triggers the same heartbeat sensation;
- whether the cough is dry or produces mucus;
- whether you are coughing blood;
- whether there is fever, wheezing or infection;
- whether symptoms are worse at night or while lying flat;
- whether you have asthma or COPD;
- whether you use albuterol or another inhaler;
- whether you take cold medicines or decongestants;
- whether you take an ACE inhibitor;
- whether there is chest pain or shortness of breath;
- whether one leg is swollen or painful; and
- whether you have known heart or lung disease.
Physical examination
Heart rate, blood pressure, respiratory rate, oxygen saturation and examination of the heart and lungs can help direct further testing.
12-lead ECG
An ECG may reveal atrial fibrillation, SVT, premature beats, conduction abnormalities or another rhythm if it occurs during the recording.
When cough and palpitations happen repeatedly but unpredictably, a Holter, patch or event monitor may help compare the cough episodes with the cardiac rhythm.
Chest and lung evaluation
Depending on the symptoms, chest imaging, asthma testing or other respiratory investigations may be appropriate.
Blood tests
Blood testing may be considered for anemia, infection, thyroid disease, electrolytes or other conditions suggested by the clinical picture.
Echocardiogram
An echocardiogram may be useful when heart failure, valve disease or structural heart disease is suspected.
Pulmonary embolism evaluation
If pulmonary embolism is clinically possible, risk assessment, D-dimer testing or CT pulmonary angiography may be used according to the level of clinical suspicion.
How Are Cough and Palpitations Treated?
Treatment depends on the cause rather than the symptom combination itself.
Respiratory infection
Treatment depends on the type and severity of infection. Fever, dehydration and breathing problems should be managed appropriately.
Asthma
Asthma treatment focuses on controlling airway inflammation and bronchospasm. If an inhaler causes troublesome palpitations, the prescribing clinician can review technique, dose and overall treatment.
Medication-related symptoms
A clinician or pharmacist can review decongestants, inhalers, ACE inhibitors and other medications that may be contributing.
Do not stop prescribed cardiovascular or respiratory treatment on your own.
Heart failure
Heart failure treatment may involve medicines, fluid management and treatment of the underlying heart condition.
Pulmonary embolism
Pulmonary embolism requires urgent medical treatment, commonly including anticoagulation and, in selected high-risk situations, more intensive therapy.
Documented arrhythmia
Rhythm-specific treatment may include observation, medication, cardioversion, catheter ablation or other therapy depending on the arrhythmia and clinical context.

When someone tells me, “Every time I cough my heart jumps,” I first want to know whether the cough started because of a respiratory illness, asthma, reflux or a medicine, and whether the heartbeat sensation occurs before or after the cough. I also ask about inhalers and cold medicines, because these can sometimes explain the palpitations. Most coughs are not caused by arrhythmias, but if the cough and skipped beats are consistently linked, a rhythm monitor can help us see whether the relationship is real. Sudden breathlessness, chest pain, fainting or coughing blood changes the situation and needs much more urgent assessment.
When Should You Make a Medical Appointment?
Arrange medical evaluation if:
- cough and palpitations keep recurring together;
- every cough repeatedly triggers a skipped or racing heartbeat;
- palpitations last longer than a few minutes;
- the rhythm feels persistently fast or irregular;
- the cough has lasted for several weeks;
- you have new wheezing or exercise intolerance;
- symptoms began after starting an inhaler or cold medicine;
- you take an ACE inhibitor and have developed a persistent dry cough;
- you develop ankle swelling or breathlessness when lying down;
- you have known heart or lung disease; or
- the symptoms are worsening or interfering with normal activities.

Do not assume these symptoms are simply a respiratory infection, asthma or anxiety.

Can coughing cause heart palpitations?

Why does my heart skip every time I cough?

Can PVCs cause a cough?

AF commonly causes palpitations, fatigue or breathlessness, but cough is not one of its most characteristic symptoms. Cough may coexist because of heart failure, lung disease, infection, reflux or another condition.
