A twitch in the pectoral muscle, a diaphragm movement and a heart palpitation can all feel like fluttering or jumping inside the chest. Some simple observations may provide clues, but an ECG or rhythm monitor is sometimes needed when the sensation could be coming from the heart.

A localized chest-wall twitch that you can see or feel beneath one small area of skin, especially if it does not consistently match your pulse, is more suggestive of a muscle fasciculation or spasm. A heart palpitation is usually perceived as an internal pounding, racing, fluttering or skipped heartbeat and often corresponds more closely with changes in the pulse. However, this comparison is not perfect: premature heartbeats can produce weak or irregular peripheral pulse waves, and some muscle movements can closely imitate cardiac sensations. If the distinction remains uncertain or symptoms recur, an ECG or ambulatory rhythm monitor can provide a much more reliable answer.

Do Not Assume a Chest Flutter Is “Just a Muscle” When Warning Signs Are Present

Seek urgent or emergency medical care if a chest flutter, palpitation or unusual chest movement occurs with:

  • significant chest pain, pressure or squeezing;
  • severe or sudden shortness of breath;
  • fainting or loss of consciousness;
  • collapse;
  • severe dizziness or near-fainting;
  • a sustained very rapid or markedly irregular heartbeat;
  • new severe weakness;
  • blue or gray lips or skin;
  • confusion or signs of poor circulation; or
  • rapidly worsening symptoms.

Chest pain and breathlessness should not be dismissed simply because you can also feel a muscle twitch.

Heart Palpitation vs Chest Muscle Spasm: What Is the Difference?

A palpitation is the sensation of being unusually aware of your heartbeat. It may feel like racing, pounding, fluttering, flip-flopping or a skipped beat.

A muscle twitch, or fasciculation, is an involuntary contraction of a small part of a skeletal muscle.

A muscle spasm or cramp is usually a stronger contraction involving more of the muscle and may feel tight, hard or painful.

No single clue proves the source.

The sensations overlap substantially. If you cannot confidently tell whether repeated chest fluttering matches your heartbeat, medical rhythm recording is more reliable than repeatedly trying to diagnose the sensation yourself.

Can Checking Your Pulse Help Tell the Difference?

It can provide a useful clue when you are otherwise stable.

While the sensation is occurring:

  1. Sit quietly.
  2. Place two fingers over the pulse at your wrist.
  3. Notice the chest sensation at the same time.
  4. Ask whether each flutter or thump corresponds with a pulse beat.

If the Twitch Does Not Match the Pulse

A visibly localized movement that continues while the pulse remains steady makes a chest-wall muscle source more likely.

This is only a clue, not a diagnostic test.

A poor pulse match does not completely rule out a cardiac cause.

A premature heartbeat may generate a weaker pulse wave that is difficult to feel at the wrist. Therefore, an ECG or ECG monitor remains more reliable than manually comparing every sensation with your pulse.

Can a Muscle Twitch Look Like Your Heart Is Beating Under the Skin?

Yes.

Superficial chest-wall muscles can twitch visibly and create the impression that something deeper inside the chest is beating.

A localized twitch may:

  • occur in one small spot;
  • look like rippling beneath the skin;
  • appear irregularly rather than at the heart’s exact rhythm;
  • occur repeatedly for seconds or minutes;
  • stop and restart unpredictably; or
  • be more noticeable while the muscle is relaxed.

MedlinePlus describes muscle twitches as fine movements caused by small, involuntary contractions in a localized muscle area.

Can the Pectoral Muscle Twitch and Feel Like a Heart Palpitation?

Yes.

The pectoralis major and smaller chest-wall muscles lie directly over the rib cage, so twitching in this area can easily be mistaken for something happening inside the chest.

A pectoral muscle twitch may be more likely if:

  • the movement is very localized;
  • you can see the skin move;
  • you recently performed chest, shoulder or upper-body exercise;
  • the muscle feels fatigued;
  • the movement occurs independently of your pulse;
  • you have been under significant stress;
  • you have not been sleeping well; or
  • you have consumed substantial caffeine or another stimulant.
Left-sided does not automatically mean cardiac.

A superficial twitch over the left side of the chest can still arise from the pectoral or intercostal muscles. Location alone cannot determine whether the sensation comes from the heart.

Why Do Chest Muscles Twitch?

Muscle twitches are common and often benign.

Common contributors can include:

Muscle fatigue

Weight training, push-ups, heavy lifting or repetitive upper-body activity can make a fatigued chest muscle twitch afterward.

Stress and anxiety

Stress can contribute both to benign muscle fasciculations and to increased awareness of heartbeats, which is one reason the two sensations can be especially confusing.

Lack of sleep

Sleep deprivation is associated with benign muscle twitching and can also contribute to heart palpitations.

Caffeine and stimulants

High caffeine intake and other stimulants can promote muscle twitching in some people while also making palpitations more noticeable.

Exercise

Strenuous exercise can temporarily produce fasciculations or muscle fatigue and can also increase awareness of the heartbeat.

Medicines or medical conditions

Certain medicines and conditions affecting nerves, thyroid function or metabolism can also contribute to muscle twitching.

Do Not Automatically Blame Magnesium

Muscle twitching or chest fluttering does not prove magnesium deficiency.

Electrolyte abnormalities can affect muscles and the heart, but taking large doses of magnesium or potassium without knowing your blood levels is not a reliable way to diagnose or treat an unexplained chest sensation.

What Is a Muscle Fasciculation?

A fasciculation is a small involuntary contraction produced when the nerve fibers controlling part of a muscle become active.

Fasciculations can be:

  • brief;
  • repetitive;
  • visible beneath the skin;
  • painless; and
  • limited to one small muscle region at a time.

Cleveland Clinic notes that benign fasciculations are associated with factors including stress, lack of sleep, caffeine, anxiety, strenuous exercise and recent viral illness.

Most isolated muscle twitching is not a serious neurological disease.

Recurrent twitching deserves medical assessment when it is accompanied by progressive muscle weakness, muscle wasting, difficulty swallowing, major numbness or other neurological symptoms.

Can a Diaphragm Spasm Feel Like Heart Palpitations?

Yes, although most ordinary diaphragm spasms do not closely resemble a cardiac rhythm.

The diaphragm is the large breathing muscle separating the chest from the abdomen.

Diaphragm-related sensations may be felt:

  • under the lower ribs;
  • in the upper abdomen;
  • at the lower chest;
  • as a repetitive flutter; or
  • as a movement linked with breathing.

Hiccups are a familiar example of an involuntary diaphragm contraction.

Exercise-related diaphragm spasms may also cause a painful “side stitch.”

What Is Diaphragmatic Flutter?

Diaphragmatic flutter, also called diaphragmatic myoclonus, is a rare movement disorder involving repetitive involuntary contractions of the diaphragm.

It can produce:

  • rapid repetitive movements in the upper abdomen or lower chest;
  • a fluttering sensation;
  • shortness of breath;
  • chest discomfort; or
  • a sensation mistaken for heart palpitations.

Rare, but a Real Palpitation Mimic

Medical literature includes patients investigated for heart palpitations whose ECG was normal and whose symptoms were ultimately found to come from high-frequency contractions of the diaphragm.

Because diaphragmatic flutter is rare, it should not be assumed to be the explanation for ordinary chest fluttering without appropriate evaluation.

How is diaphragmatic flutter diagnosed?

In selected patients, clinicians may use:

  • diaphragm ultrasound;
  • fluoroscopy;
  • electromyography (EMG); or
  • other respiratory or neurological testing.

Ultrasound has been described as a useful noninvasive method for showing the diaphragm itself as the source of abnormal repetitive movement.

Could It Be an Intercostal Muscle Spasm?

Yes.

Intercostal muscles lie between the ribs and help move the chest during breathing.

Strain, fatigue or spasm in these muscles can produce:

  • a localized twitch;
  • pain between the ribs;
  • tightness;
  • a fluttering sensation; or
  • discomfort that changes with movement or deep breathing.

A localized chest-wall sensation that changes when you rotate your torso, contract the chest muscles or press on a tender area is more suggestive of a musculoskeletal source than a rhythm problem.

But Movement-Related Chest Pain Does Not Completely Exclude a Heart Problem

Chest pain should be interpreted in the full clinical context. Significant pressure, breathlessness, fainting or other emergency symptoms require medical evaluation regardless of whether some tenderness is also present.

Which Heart Palpitations Can Feel Like a Muscle Twitch?

Several cardiac rhythm sensations can be very brief and localized enough to feel like a chest muscle moved.

PACs

Premature atrial contractions are early beats that arise in the atria. They may feel like a skip, flutter or sudden thump.

PVCs

Premature ventricular contractions begin in the ventricles. The early beat may be followed by a pause, making the next heartbeat feel particularly forceful.

The American Heart Association notes that this strong post-pause beat is often what patients interpret as the heart having “skipped.”

SVT

Supraventricular tachycardia usually produces a more sustained, suddenly rapid heartbeat rather than one isolated surface twitch.

Atrial fibrillation

AF usually produces an irregular rhythm that may feel fluttering, quivering or chaotic rather than a single localized muscle jump.

A palpitation can also occur during completely normal sinus rhythm.

Heartbeat awareness does not automatically mean an arrhythmia. Outpatient monitoring sometimes shows normal electrical rhythm at the exact time a patient reports a palpitation sensation.

What if the Chest Twitch Happens Every Few Seconds?

Repetitive symptoms can occur with either muscle twitching or heart rhythm disturbances.

If you notice:

  • one visible surface twitch repeatedly;
  • a stable wrist pulse while the surface movement continues; and
  • no dizziness, breathlessness or other cardiovascular symptoms,

a muscle source becomes more likely.

On the other hand, a repeated skipped/thumping sensation corresponding with irregular pulse beats may represent frequent PACs or PVCs.

What if the Twitch Is on the Left Side of My Chest?

The left side of the chest contains both the heart beneath the rib cage and several layers of chest-wall muscle above it.

Therefore:

left chest = heart

is not a medically reliable rule.

A superficial pectoral twitch may occur directly over the area where people expect the heart to be, while genuine palpitations may sometimes be perceived centrally, on the left side, in the neck or even in the throat.

Can Anxiety Cause Both Muscle Twitching and Heart Palpitations?

Yes, which is one reason the distinction can be difficult.

Stress and anxiety are recognized triggers for benign muscle fasciculations.

They can also increase:

  • heart rate;
  • force of cardiac contraction;
  • adrenaline levels; and
  • awareness of internal body sensations.

A person may therefore have a genuine pectoral twitch and heightened heartbeat awareness at the same time.

Can Lack of Sleep Cause Both?

Yes.

Lack of sleep is associated with benign muscle fasciculations and is also a recognized contributor to premature heart contractions.

This means someone who has been sleeping poorly may experience both:

  • surface muscle twitching; and
  • actual PACs, PVCs or increased heartbeat awareness.

Can Dehydration or Electrolyte Problems Cause Both?

Significant fluid and electrolyte disturbances can affect both skeletal muscle function and cardiac electrical activity.

However, a chest twitch does not tell you which electrolyte is abnormal.

Situations that make testing more relevant include:

  • prolonged vomiting;
  • significant diarrhea;
  • heavy prolonged sweating;
  • diuretic use;
  • kidney disease; or
  • other reasons for substantial fluid or electrolyte loss.

Could a Flutter Under the Ribs Be My Heart?

Not necessarily.

Sensations beneath the lower ribs or upper abdomen may arise from:

  • diaphragm movement;
  • abdominal-wall muscle twitching;
  • normal awareness of the abdominal aortic pulse;
  • gastrointestinal movement; or
  • heartbeat awareness transmitted through nearby tissues.

How Do Doctors Tell a Muscle Twitch From a Heart Palpitation?

The evaluation usually starts by determining whether the sensation correlates with the electrical heartbeat.

History

Your clinician may ask:

  • exactly where the sensation occurs;
  • whether it is visible;
  • whether it matches your pulse;
  • whether it feels deep or superficial;
  • whether it follows exercise or muscle fatigue;
  • whether movement or posture changes it;
  • whether it is linked to breathing;
  • how long it lasts;
  • whether it begins and stops suddenly;
  • whether the pulse feels irregular;
  • caffeine and stimulant use;
  • sleep deprivation;
  • stress and anxiety;
  • medicines and supplements; and
  • whether dizziness, breathlessness or fainting occurs.

Physical examination

A clinician may observe the chest wall while checking the pulse and listening to the heart.

A visible muscle movement that occurs independently of the cardiac rhythm can help identify a musculoskeletal source.

12-lead ECG

If the sensation occurs during an ECG, the tracing can show whether the electrical rhythm changes at the same time.

A normal ECG recorded after the symptom ends is less conclusive.

An intermittent PAC, PVC, SVT or other rhythm may disappear before the ECG is performed. Recurrent symptoms may therefore require longer monitoring.

Holter, patch or event monitoring

Ambulatory ECG monitoring can compare your reported symptom with the electrical rhythm over hours, days or longer.

This can answer a very useful question:

“Was the heart rhythm abnormal at the exact moment I felt the twitch?”

Muscle or neurological evaluation

If the movement is clearly muscular and persistent, evaluation may include:

  • neurological examination;
  • review of medicines and stimulants;
  • thyroid or electrolyte testing when appropriate; or
  • EMG in selected cases.

Diaphragm imaging

Rare suspected diaphragmatic myoclonus may be evaluated with diaphragm ultrasound, fluoroscopy or EMG.

Can a Phone Video Help?

Sometimes.

If there is a visible, repetitive chest-wall movement, a short video recorded during the episode may help your clinician understand what you are describing.

It may be especially useful to show:

  • the exact location of the movement;
  • whether the skin visibly twitches;
  • whether it occurs with breathing; and
  • how long the episode lasts.

A Video Does Not Replace an ECG

A phone video can document a surface movement, but it cannot reliably determine the heart’s electrical rhythm. When a cardiac cause remains possible, ECG monitoring is the more appropriate diagnostic tool.

When someone tells me, “Something is twitching over my heart,” I first ask whether they can actually see the movement and whether it matches the pulse. A tiny visible movement in one part of the pectoral muscle while the wrist pulse stays regular suggests something very different from a deep thump that occurs with every irregular pulse beat. But I do not rely on that observation alone. Premature beats can sometimes produce weak pulse waves, and rare diaphragm movements can imitate palpitations surprisingly well. When the sensation keeps recurring or the patient cannot confidently tell where it comes from, recording the heart rhythm during the symptom usually gives us a much better answer.

When Should You Make a Medical Appointment?

Arrange medical evaluation when:

  • you cannot tell whether the sensation is a muscle twitch or heartbeat;
  • the flutter keeps returning;
  • the pulse feels irregular during the episode;
  • palpitations last several minutes or longer;
  • symptoms are becoming more frequent;
  • the sensation occurs during exercise;
  • there is dizziness or unusual weakness;
  • you experience unexplained shortness of breath;
  • you have known heart disease or a previous arrhythmia;
  • muscle twitching becomes widespread or persistent;
  • twitching occurs with progressive muscle weakness or wasting; or
  • the symptoms are interfering with normal activities.

  • severe or sudden shortness of breath;
  • fainting or loss of consciousness;
  • collapse;
  • severe dizziness or near-fainting;

  • new severe weakness; or
  • rapidly worsening symptoms.

Can a chest muscle spasm feel like heart palpitations?
